2. The proposed work consists of approximately 78 SY of pavement repairs 107 SY of pavement planing and 114 CY Asphalt Concrete Resurfacing on Oak, East & Elm Streets in the Village of Belmore, Ohio.

3. Engineer’s Project Cost Estimate is $38,000.00

4. The Bidding Documents may be purchased at a cost of $25.00 (non-refundable). Requests for bid packages shall be directed to Bockrath & Associates Engineering and Surveying, LLC, 115 S. Fair Avenue, Suite A, Ottawa, Ohio 45875, Phone: 419-523-5789.

5. Bids must be submitted on the unbound Bid Forms furnished in the Bidding Documents and must be accompanied by a Bid Guaranty and Contract Bond in accordance with Section 153.54 of the Ohio Revised Code. Bid Security furnished in Bond Form, shall be issued by a Surety Company or Corporation licensed in the State of Ohio to provide said surety.

7. This project is funded by the OPWC Infrastructure Grant Program. Attention must be given to all the requirements contained in the bid packet, prevailing wage rates, various insurance requirements and the requirement for a payment and performance bond for 100% of the contract price.

8. Bidders must comply with the prevailing wage rates on public improvements in Putnam County, Ohio as determined by the Ohio Bureau of Employments Services, Wage and Hour Division.

9. Bids will only be accepted from the Bidders listed on the Engineer’s Official plan holders list.

10. The Village of Belmore reserve the right to reject any or all bids, and to waive all informalities not involving price, time, or changes in the work.
